I-134 Affidavit of Support
If you need to show that you can financially support your relatives (or anyone else) visiting the U.S., you have to fill out an I-134, Affidavit of Support form. You do not have to get it notarized.
- Print it on 1 page double-sided. Do not print it on 2 pages. If you do not have a printer that prints double-sided, first print the 2 pages individually and then make a 1–>2 photocopy. Another option is to print the first page, reverse it and stick it back into the paper holder, and then print the second page.
- This form does not need to be notarized.
- Many people ask whether the sponsor is liable for any medical expenses that visitor medical insurance may not cover due to pre-existing conditions.
